If you are age 18 to 59 and have a disability as defined by Social Security, you may apply for a card at most public libraries and senior centers.  Due to ID, age, and disability verification requirements, you cannot apply for the card by phone or online.
   
All Ohioans age 60 or better, who hold current Ohio Driver Licenses or State ID cards automatically receive the Golden Buckeye Card in the month of their 60th birthday.
